We request that any families who would like to make Holy Name of Jesus their parish home please fill out the Parish Registration Forms:

Registration forms are located next to the Parish Office window in the Administration Lobby.
If you have any trouble locating the Parish Office during the weekend, please ask a
Minister of Hospitality to help you.
Print Registration Forms Online
Download and print the Registration Forms (lower left hand corner).
Turn In Your Forms
Please turn in all Registration forms to the Parish Office during normal weekday Business Hours: (M-Th, 8:30am - 3:30pm) OR give them to a Minister of Hospitality during the weekend.
